- Log into ptk.org
- Click on Advisors
- Scroll down to Programs
- Click on Five Star Chapter Plan
- Click on Report Your Progress
- In the drop down Menu, select your chapter
- You should only see your one chapter unless you are an advisor of multiple chapters
- Check off items that you have completed.
- This form only shows items that you are allowed to check off. The items that Headquarters checks off are not on this form.
- For a full list of requirements go to Advisors > Programs > Five Star Chapter Plan > Level Requirements.
- For a list showing what you and Headquarters have marked as complete go to Advisors > Programs > Five Star Chapter Plan > Progress Reports > Click on Your Region > Click on Your Chapter.
